District Nurse Services
The District Nursing Service aims to provide care to people with complex needs at or as close to home as possible in order to prevent avoidable admissions to hospital and to facilitate the earlier discharge of patients. (Care closer to Home).
The service operates an 'open referral' system and responds to requests from other professionals, cares, family or self referral. They work in partnership with patients, their carers and other statutory and voluntary agencies to ensure, wherever possible, that patients are cared for in the primary health care setting.
