I need an assessment

Adult Social Services helps the following people who live in Trafford and are aged over 18:

  • People with mental health issues
  • People who have a physical disability
  • People who have a health condition
  • Older people
  • People with a learning disability
  • People who care for someone

If you are finding it difficult to manage at home, are caring for someone and need some support, or are concerned about someone who may be at risk then contact Trafford Adult Social Services. We will work with local health providers, voluntary organisations and/or private companies to help you get the support you need.

If you get support after an assessment you may need to pay towards the cost of your care.

 

Your assessment

If you are finding it difficult to manage at home you can ask for an assessment.

 

Your reassessment

It is important that you are reassessed regularly to find out if the support you need has changed.

 

Your carer's assessment

As a carer you have a right to have your needs considered when decisions are being made about support for the person you care for. 

 

Equipment and adaptations

If you are finding it difficult to manage at home there may be specialist equipment to help you.
