Community Heart Failure Specialist Nurse Service
This is a Nurse-led service that helps to improve the condition of patients with heart failure caused by Left Ventricular Systolic Disfunction (LVSD) problems.
The service includes initiation and titration of medication, and education to enable patients to manage their condition and alert the HF team when their condition is becoming unstable.
