The CAN Social Care team carry out child and family assessments to identify unmet social care needs and work to determine if these needs can be met through the family, specialist support or universal services.
The Social Care Team are a statutory body within the Multi–agency Complex and Additional Needs Service and have a statutory obligation to safeguard children and young people with complex and additional needs and provide a duty of care to children and their families.
If a child or young person is eligible for support from the team, following assessment, they will be awarded a personal budget which can be developed in accordance with individual need such as the employment of a PA to develop socialisation opportunities for the child. Social workers will then continue to support the family if there are needs that require the intervention of a social worker.
The team also hosts the Home from Home Short Break Scheme, Family Support Workers and a Senior Family Support Worker whose role is to review personal budgets with families in line with the personalisation policy.

- Neuro-Developmental Pathway
- Trafford Sensory Impairment Service
The Social Care Team work closely with these other professionals to ensure positive outcomes for the child and to ensure the voice of the child is considered at all times.
