MFT Care Navigation Service
The service, based in Wythenshawe Hospital, supports Trafford residents to navigate health and social care services. The Care Navigators help residents to connect with services, organisations and activities in their communities. The service offers support over an 8 week period and anyone living in Trafford can access the service. Residents can either self refer or be referred by a professional.
