Community Matrons
Community Matrons are advanced practitioners with non medical prescribing qualifications who manage patients with complex or multiple long term conditions in their own home to reduce frequent admissions to hospital and facilitate the earlier discharge of these patients.
Community matrons draw up a management plan in consultation with the patient and their carers with the aim of early recognition and action to prevent exacerbation.
The Matrons work in conjunction with other health and social care professionals and the voluntary sector, providing advice, support and education to the patient and their formal and informal carers.
