Trafford Children's Community Nursing Team

The Trafford Children's Community Nursing Team (CCNT) are a team of qualified children's nurses and support workers who are dedicated and committed to delivering care closer to home for children and young people within the Trafford area. The main aims of the CCNT service are to:-

  • Deliver care closer to children and young people who have mild acute illnesses within their home or other community settings.
  • Reduce hospital admissions and length of stay for children with ongoing and chronic conditions
  • Provide support within the homes for children and young people with complex and additional health needs
  • Reduce avoidable hospital admissions for children's with acute illnesses by working in partnership with primary, secondary and tertiary care
  • Facilitate early discharge from hospital following elective and non elective surgical admissions
  • Provide accessible nurse led clinics within the community setting
  • Provide education and training to families and our CYPS multi-agency colleagues involved in providing care to the children and young people of Trafford  
  • Promote health and well being for all children and young people in Trafford by reducing health inequalities for those who are most vulnerable.

The CCNT provides a range of nurse led clinics.  The current provision is for the following conditions:-

  • Continence (nocturnal bed wetting): Tuesdays 2pm to 4.30pm and Thursdays, 2pm-4.30pm
  • Eczema clinic: Mondays  1.30pm - 4.30pm, Wednesdays, 2pm-4.30pm

 
The CCNT aims to promote self care and health promotion for the children and young people of Trafford, by working proactively where possible, and being reactive to their individual needs. 

Parental and children/young people's empowerment is one of the fundamentals of children's community nursing and is encouraged by the support, care and advice that the CCNT provide.

We also work in partnership with our Children and Young Peoples Service colleagues to promote and enable all children and young people to achieve their best possible outcome.

Referral Details

Referrals to the CCNT can be made via telephone, letter or fax and are accepted from all agencies working with children and young people including self referrals.  Any child or young person with a nursing need can be referred to the CCNT. Some examples are:

  • Support for children and young people with complex and additional needs.
  • Post operative care
  • Care during acute illnesses
  • Wound care - general, post operative, burns/scalds, suture removal
  • Enuresis management
  • Constipation management
  • Acute asthma care
  • Orthopaedic care
  • Respiratory care
  • Rheumatic conditions requiring subcutaneous medications
  • Chronic asthma management
  • Eczema or other skin conditions management
  • Oncology - support and chemotherapy administration
  • Palliative and end of life care
  • Epilepsy support
  • Cystic fibrosis support - also give courses of intravenous antibiotics when necessary
  • Phlebotomy for children with complex and additional needs
