GP, Health professional or Social care professional
If you are in hospital your needs will be assessed by a therapist and they will refer to us for equipment or minor work to be actioned, in order to facilitate a safe discharge. Depending on your needs, the hospital may request a further assessment in your own home following discharge from hospital by the Assessment Team at the One Stop Resource Centre.
Community Nurses can assess your complex health needs at home or in a treatment room setting and refer any equipment needs to us for action.
Referrals can be made via: mft.admin.osrc@nhs.net
Self-referral:
If you are a Trafford resident you can self refer to us or ask a family member or friend to do this on your behalf by phoning our Equipment and Adaptations Advice Line on 0345 299 0798.
The telephone is staffed by a member of the occupational therapy assessment team. They will take your details and advise you of possible options which may include arranging an appointment for a home visit/assessment. Appointments are allocated on a priority basis and it may be some time before an occupational therapist can visit you. Please ask when calling how long the current waiting time is.
