Continuing Care - Children's Personal Health Budget
An NHS continuing care package may be offered when a child or young person under 18 years of age has needs arising from disability, accident or illness that cannot be met by existing mainstream / universal or specialist services alone but require regular and planned complex intervention.
This extra help can provide an improved quality of family life and extra support for your child, you and your family.

Referrals can be accepted from any health or social care professional, who thinks a continuing care package may be needed. As a parent / carer if you feel your child may be eligible, please contact myself, your Children’s Community Nurse or your Social Worker for more information.
