Transitional Plus Care - Community Interventions
Our Interventions Pathway work will be delivered in a multi-agency approach using the skills of a wide range of sessional professionals and partner agencies who will take individuals on a journey of reflection and self change.
The partner agencies and sessional professionals who have been identified to work with Transitional Plus Care have been so on the basis that they will be able to continue to work with individuals in an integrated approach through mainstream services in community.
Transitional Plus Care is committed to safeguarding and promoting the welfare of young people. Our pledge and core business is to thoroughly screen all employees, partner organisations and agents. We carry out rigorous checks which comply with Care Quality Commission (CQC) safeguarding code of professional practice.
