Together Trust Trained Carers

The Together Trust will continue to provide trained carers for short breaks, during the day, in the evenings, at weekends and in the school holidays, both in and out of the home. This service will work exclusively with families with children with complex needs and who require specialist support and provide trained support on a one to one basis (2:1 support in agreed cases) in and out of the home and in the community.

The family based short break services we provide are staffed through our Community & Domiciliary Services based in Cheadle and are complemented by the use of volunteers. We have designed and developed our services to meet the needs of the following groups of children and young people aged 0 to 19 years with:

  • Autism & Asperger’s conditions including those whose behaviour challenges traditional services
  • Physical and/or learning disabilities
  • Communication and/or sensory impairments
  • Complex health needs requiring specialist support
  • Palliative care needs
  • Complex and additional needs who are in transition

Referral Details

In accordance with Trafford's Compex and Additional Needs Short Breaks Eligibility Criteria, targeted provision would be for young people whose lives are profoundly affected by one or more of the criteria set out in the eligibility details section.
