Patient Transport Service
You may be eligible for the Patient Transport Service (PTS) if you have a medical condition which could stop you getting to your appointment by any other means.
In Trafford Patient transport is organised by the Trafford Care Co-ordination Centre. When you telephone, you will need your NHS number, which you can get from your GP or clinic. You will be asked about:
- your medical condition and your mobility.
- the date, time and place of your appointment.
- if anyone will be going with you
The questions will be asked every time you need to book transport unless you are a renal dialysis or cancer patient.
If you are not eligible you you will get information other ways to get to your appointment and information on how to get help with travel costs.
