Children's Learning Disability Nursing Team (CLDNT)

Who we are:

The Children's Learning Disability Nursing Team provide support services for Children and Young People up to the age of 18 who have a severe learning disability and/or additional and complex needs such as behaviour which is challenging to manage.

The team support children and young people and their families within their own homes and provide a range of interventions dependent on individual need.

We are a small team consisting of Community Learning Disability Nurses and a Community Support Worker. We work in conjunction with Clinical Psychologists in the Learning Disability CAMHS service and often complete joint assessments and intervention plans.

What we assess:

Assessments are completed in relation to behaviour, sleep, continence or other health needs. Programmes of intervention are offered to promote positive outcomes and to support children and their families from both a practical and emotional perspective.

During an Initial Assessment, staff will discuss the different ways in which support can be offered and a plan is made in partnership with parents/carers.

The team regularly work in partnership with colleagues in other agencies and promote a multi-agency approach to working.

As part of the assessment process, the team regularly complete visits to nurseries/schools to assess children in different environments and to promote a consistent approach to strategies employed.

What support we offer: 

Where appropriate we offer individual 1:1 support to young people to promote their understanding of emotions and their ability to self-regulate. These sessions can take place either in home or school environments depending on individual need.

The Team offer Group Interventions to parents/carers. These are a positive way to learn new strategies whilst sharing ideas with others within a group format. The ‘Incredible Years’ is an evidence-based Positive Parenting Course which has been adapted to meet the needs of children with additional needs.

‘Riding the Rapids: Living with Autism or Disability’ is an eleven session parent-training course for parents and carers of children and young people with a diagnosis of Autism or other complex needs.

Where referrals to the team include working with young people of 14 years or year 9 at school, we will liaise with other relevant professionals regarding transition planning.  

Continence Support:

The Team aim to support and improve the health of children and young people by promoting independence with continence where possible.

If it is agreed that a child/young person is ready to follow a toileting programme, this will be designed by Community Learning Disability Nurses in conjunction with the child and their parents/carers. An individualised approach is adopted and each step designed to meet the developmental needs at that time.

Following assessment, if it is agreed that the child/young person is not ready to develop and promote Continence, a referral is made to the Bladder and Bowel service. This will be for the child/young person to be placed on the self-care model to receive a regular delivery of Continence products (Pads). 

A further referral to the team would be welcome if the child/young person begins to show continence awareness and additional support is required.

Our main office working hours are between 9am and 5pm Monday to Friday however we regularly work outside of these hours in order to respond to the individual needs of children and families within their home environment.

In order to meet the criteria for referral, a child or young person will;

  • Be under the age of 18 years
  • Be resident in Trafford or be registered with a Trafford GP
  • Have a severe learning disability
